We report a case of bilateral large aneurysms of the cavernous internal carotid artery treated by reconstruction of the bilateral ICAs with radial artery grafts and trapping of the ICAs. Bilateral radial artery grafts provide sufficient cerebral blood flow and good vascular reserve.
