We evaluated pyramidal tract of hemiparesis patients by DTI, which demonstrates continuity of pyramidal tract. Methods: Ten patients with intracerebral hematoma were studied with DTI in four millimeter thickness coronal sections parallel with the pyramidal line since July, 2002 to October, 2002. We divided patients into two groups based on their continuity of pyramidal tract. Group A consisted of five patients with continuous pyramidal tract. Group B consisted of five patients with discontinuous pyramidal tract. We compared the improvement of Manual Muscle Testing (MMT) between two groups. Results: In a group A, MMT of upper and lower extremities improved from 2 / 5 and 2.2 / 5 to 4 / 5 and 4.4 / 5, respectively. In a group B, MMT of upper and lower extremities did not improve from 1 / 5 and 1.2 / 5 to 1.6 / 5 and 1.8 / 5, respectively. There was a significant statistical difference of MMT recovery between two groups.
