Loss of visual acuity and painful ophthalmoplegia were appeared inspite of steroid therapy. The angiography revealed the findings of occlusion of the right ophthalmic artery and stenosis of the right internal carotid artery at the cavernous portion. In this patient,cavernous dural inflammation was associated with a histologically identical spinal lesion. They were same pathological specimen,showed marked hypertrophy of the dura mater and infiltration of numerous lymphocytes and plasma cells. The etiology and management of the condition are discussed.
