In animals, this vascular network is a normal anatomical structure, but in man, it is extremely rare. We reported a case of 58 -year-old man, in whom the presence of bilateral rete mirabile with bilateral internal carotid arteries occlusion was demonstrated by angiography. Hemodynamic reserve in this patient was indicated normally using single-photon emission computed tomography (SPECT) and N-isopropyl123 I-p-iodoamphetamine(IMP).
