It is never other people’s affairs. Based on this situation, this paper aims to study the uniqueness of care work as the preliminary study of “Jiritsu” (including independence and autonomy) and vulnerability in home care which attempts to enable people to live their own lives to the end. This paper includes actual examples of in home-visit long-term care from my own experiences as a certified care worker. The study is carried out from three points of view: the terminology of “Kaigo (care work)”, expertise of care work and history of home care. The term “Kaigo,” which means care work, appeared in a notification about the pension of the army for the first time in 1892. “Kaigo” was used as a substitute for the term “Kango,” which means nursing, and that came to be used as a medical technical term. It is hypothesized that “Kaigo” was formed from “Kango” and “Kaihou,” which means looking after in Japanese. Care work is never a substitute for medical assistance. The work of a caregiver is not intended to make up for the lack of appropriate medical care, but instead, it is to provide appropriate care considering the remaining function and willingness of the recipient. Care work is never medical care. Instead, the approach is at the social level, providing support for daily living. Modern Care work doesn’t mean caregiving in the private sector, such as taking care of one’s family, rather care-welfare is expert socialized care. Recipients are most comfortable at home. So home care is designed to respect the recipient’s daily life. The first public home help service in Japan was started in Nagano prefecture in 1956. The caregiving movement developed on a nationwide scale and led to the establishment of home help service programs for the aged by the Ministry of health and Welfare. Following the Act on Social Welfare for the Elderly in 1963; it bore fruit as home help service. In 1987, programs to certify caregivers were established. The Long-Term Care Insurance Act was passed in 2000 and home-visit long-term care spread throughout the country.
