journal","bibliographic_titleLang":"en"}]}]},"item_5_description_4":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"腰痛を生涯に経験する日本人は約8割存在し,その原因には様々な疾患が存在する1)。腰痛には先天的なものから成長に伴い発生するもの,加齢によるものや感染,腫瘍により発生するもの等が考えられる。 この中で,下肢にしびれや痛みの症状を訴える患者は腰椎椎間板ヘルニアや腰部脊柱管狭窄症などによる神経圧迫の可能性がありmagnetic resonance imaging(MRI)検査による診断が有効である2.3)。しかし,画像上異常所見がみられない場合には手術や神経根ブロックなどの治療を受けることができず,腰痛持ち患者を悩ませる場合がある。これらの場合に他の要因が考えられ,その一つとして外側ヘルニアを疑う指標となる。腰椎椎間板ヘルニアの中でも外側ヘルニアの発生頻度は,全腰椎椎間板ヘルニアの7〜12%以下と比較的稀である4)。デルマトーム5)に従った下肢の身体所見があるにも関わらず,脊柱管内にヘルニア等がみられない場合,従来の撮像では見落としの可能性がある。そのため,MRI 検査方法も追加撮像等工夫が必要となる。近年MRI 装置の発展により,MR ミエログラフィが広く行われている3)。一般的にMR ミエログラフィはfast spin echo 法(FSE)などを用いたHeavyT2強調画像を用いるが,従来の撮像方法ではcerebrospinal fluid(CSF)の描出が強調されている。しかし腰神経末梢まで描出されないことや背景信号が少ないため詳細な神経の圧迫状況や圧迫原因の同定は困難となる事が多い。今回検討を行ったdouble echo steady state 法(DESS)6,7)は,fast imaging with steady state precession(FISP)とtime reversed FISP(PSIF)の二つのエコーを取り込むシーケンスである。DESS 法はFISP 信号がもつ解剖学的情報とPSIF 信号がもつCSF 等の水成分を同時に描出可能である。本法は,関節領域の撮像に開発されたシーケンスであるが,神経根の圧迫原因や病態の状態把握をより明瞭にできるのではないかと考え,腰椎三次元冠状断撮像(3Dimension coronal scan)に応用し,他の類似シーケンスであるsampling perfection with application optimized contrasts usingdifferent flip angle evolution法(SPACE)8)やTrue Fast Imaging with Steady State Precession 法(True
