Extradural temporopolar ap-proach (EDTA) with this procedure makes it possible to preserve venous return from superficial middle cerebral vein (SMCV). We evaluated the variation of venous system with preoperative angiography in 24 cases and in 9 of 24 cases checked post-operative angiography also. EDTA did not make venous return from SMCV worse in any cases. Preoperative venous route from SMCV occluded in most cases ofCavernous sinus type (CS type) in which SMCV join cavernous sinus directly or through spheno-parietal sinus. But no cases of CS type had venous infarction.
