内容記述 | Purpose: To elucidate possible clinical factors affecting visual field (VF) deterioration in patients with primary open angle glaucoma (POAG).Patients and methods: 47 eyes of 29 POAG patients who were or were not receiving glaucoma edications were included. Each subject underwent VF tests 3 or more times using the Humphrey Field Analyzer 30-2 (HFA30-2). Based on their scores for three patterns of classification - (1) Advanced Glaucoma Intervention Study visual field defect score (AGIS score), (2) mean deviation slope (MD slope), (3) both AGIS score and MD slope - patients were divided into a progressive group and non-progressive group. Intraocular pressure (IOP) measurements at 14 time points (12, 15, 18, 21, 0, 6, 9, 12, 15, 18, 21, 0, 6 and 9 o’clock) were performed over a period of 48 hrs. IOP fluctuations were classified into 4 patterns (diurnal acrophase, nocturnal acrophase, flat and double acrophase). Several clinical factors between the two groups, including age, sex, central corneal thickness, visual acuity, refraction, the number of anti-glaucoma medication, mean IOP, maximum IOP, minimum IOP, IOP fluctuation and circadian IOP fluctuation patterns were compared. Results: Patient age and a nocturnal acrophase pattern were found to be significant factors related to the progression of VF deterioration. Patients in the progressive group were significantly younger than those in the non-progressive group in the MD slope classification (odds ratio = 0.913, P = 0.013). Nocturnal acrophase circadian patterns were more frequently observed in the progressive groups classified with AGIS score and both of AGIS score and MD slope. Conclusions: These findings suggest that age and nocturnal acrophase patterns are possible clinical factors related to the progression of VF deterioration in patients with POAG. | |||||
