| 内容記述 | 今日、がんゲノム医療や新薬の開発に代表されるように、医学・医療は急速に高度化している。他方、現代の医学・医療において、患者/被験者の人権の保障等医療・医学研究の適切さを担保する大きな役割を倫理審査委員会が担っている。倫理審査委員会においては、専門家の専断を防ぐ目的から、「一般の立場」を代表する委員を配置しなければならない。しかし、高度化する医学・医療について一般の立場の委員が適切に審査することは容易ではない。このような課題を解決するためには、個々の診療や研究の場面を超えて、医学・医療界と患者・市民が医学・医療について日常的に意見交換する場を設定することが有意義である。今日、そのメソッドとして、Patient and Public Involvement（PPI）が本邦に導入され、普及が試みられている。本稿では、がんゲノム医療と新薬開発の現況をレビューするとともに、それらの高度な医学・医療を適切に実施するための患者・市民の関わり方、また倫理審査のあり方について考察した。 The medical science and clinical practice concerning cancer genomic medicine and new drug development has been rapidly developing. Concurrently, ethics committees or institutional review boards play important roles to ensure the rights of patients/research subjects and the quality of both clinical practice and medical research. Ethics committees and institutional review boards must include at least one layperson. However, reviewing the advanced medical protocol appropriately can be a challenging task for these members. In order to resolve this issue, it is important to have fruitful informal discussions between those involved in academia, patients and the committee/review board laypersons. The Patient and Public Involvement (PPI) has been introduced in Japan as a means to facilitate such discussion. This study will review the present state of cancer genomic medicine and new drug development, and consider methods of ethical review, including the manner in which patients and layperson committee/review board members are involved. |
